What do you consider to be the most annoying in the game? As for me:
-snake pits
-stone traps
-"tension rooms" with vortices in the cute dog cave
-aging ( especially being aged by 20 yrs. by o ghost bat )

Anyways, here's some more:
- Greater undead vaults
- Black dragons and all the other acid things
- Corruptions
- Rivers in dungeons(if not playing a spell caster)

Mixed vaults/tension rooms. WAY tougher monsters than the ones with a specific set of monsters. Rivers can also be annoying in the early game. Of course killing a cat sucks. That's about it.

-Players (mine) own stupidity
-New spells change the places of old ones
-Stupid stoneblock traps coming out of nowhere (spellcasters mostly)
-Getting ambushed in the wilderness and then surrounded with lowlevel monsters who slowly rip you apart (you know, half of the screen full of jackals...)

Technically, it's cheating, but anyways.....
check out cheatinginadom, download GenTrain and the ADOM-Sage cheatlist (adom-sage is the only platform where the values stay constant), and reverse it.
You can go back to whatever version you were playing, as I have found no difference in the saves between ADOM-Sage and ADOM (DOS). I'm not sure about the WinBeta4.

It works great, I've saved probably 4 charecters. It doesn't work if you do it on the cat lord level though. :-0.

I hate it when my climbing set breaks or is dissolved somehow once I cross the mountains to go onto the RotHK cave in the northwest region. You cant get back over, it sucks.

So I have to hang out and level up til climbing is high, go in the rift to inexperienced, and die on my way to the ratling trader who wants to sell me a climbing set because I got paralyzed by some master mimic. (That happened last night, still bitter, with my drakeling merchant.) I think merchants find more shops.
