Is there some secret that I am missing regarding the ability to smith really good weapons and armour? The best I can ever get is maybe 3 or 4 improvements on any individual item (which isn't bad, I just would like more). I have smithing skill at 100, metallurgy skill at 100, concentration skill at 100, wear a leather apron, and wield an eternium warhammer when working on items. After the third or fourth improvement, all I get is "You fail to improve the *whatever*". HELP! PLEASE!

Just try. And try. And TRY EVEN HARDER. I noticed than iron items are easier to improve than those of higher metals, but nevertheless I had elven chain mail [+8, +7], metal cap [+7, +7], etc.

The weaponsmith class has a great potential, in the mid-game a decent weaponsmith can be almost untouchable. And my greatest achievent was a sword of sharpness (+9, 4d8+10). Impressive, isn't it? Find a better 1-handed weapon :) (excluding S&N, they work only with 2 hands occupied).
