I've done all the beginnig quests (rescued the cute dog, kill Kranach, rescued the carpenter, kill Hotzenplotz). Since I didn't found an important item, I did the normal ending, and then kept on playing in the SMC and the infinite dungeon. I had to go deep in the ID for a quest (lvl 334), so when I came back later, I try to find staircases quick to go deeper. I was on lvl 172 when I met a doppleganger king (in fact, I was playing so quick he hit me before I see him). I had only ~500 HP left then (1425 normally), and for some unknown reasons, I did not withdraw to heal me, but I took my sthetoscope to examine him.
