What decides whether you can see a map of the level you died on? Because sometimes you can, and sometimes you can't. And if there's something you can change so you can always see the map, I'd like to know.

I cant think of an occaision where I have not been offered to see the entire level after the PC has died...I play winbeta 1.1.1

I thought this was a standard feature.

They're both special locations. Only on normal dungeon levels are you allowed to see the level you died on.
