I always saved Khelavastar mainly because I got to level 20 only twice. At level 20 I usually have a wish or an AoLS

This happened with my first char who ever won the game (a gnome priest).

The scenario: The gnomish priest is dropping off his goodies in his locked store room above dwarf town. He want's to be extra careful with the items and doesn't want them to get destroyed in his many journeys. The goodies include such precious items as three SoCR, two PoGA, misc. Potions of attributes, one extra pair of seven league boots etc. Suddenly the priest hears a distant wind. Thinking nothing of it, he finishes his business in the storage room and opens the door...

A fire vortex... and the little precious things are only one step from it. Had I not been so cocky (read stupid) I could have teleported it, or myself, away from the precious things. But I trusted my frost bolt to be effective enough. It wasn't. Boom! Something on the ground is burned to ashes, something on the ground is shattered to pieces.

Oh well, you live, you learn.

Dammnit just lost a potentially awesome Drow ranger to a kobold. Was born on Candle with long lost brother random effect, found an altar when I was lvl 9 in the VD, converted it to chaotic, blessed water, ided some items, began saccing, then kobold sneaks in while I am not looking and sacs me. Adom should just realize I am too frustrated with it and too stubborn to back down and just let me win because it is only delaying the inevitable...
Tell me how to get stronger....

I made it to the Casino, picked up the Gift Shoppe via SoA, read a blessed potion of identify, and dropped the loot for easier perusal. Seeing interesting items, I went to the slot machines to earn the dinero. One minor problem: the shopkeeper says I owe him $137862 or something equally ridiculous. Checking my corruptions, I notice belatedly that I absorb mana. Having no way to pay or steal, I attacked the shopkeeper. He killed me instantly.

Another terrible blunder by yours truly, not quite up to the sad end of my archmage but still...

A decent char, gnomish wizard, having achieved all elemental orbs bar the mana one. In the mana temple I summoned a familiar, getting a greater earth elemental. Letting my companion take care of the servants (not that I could've done it myself, just wanted to save some bother and have my pet soften up NV before finishing him up myself).

After a while my pet has killed a lot of servants and NV decides to confuse my char. No problem, just swap some equipment to raise willpower. Having swapped fire orb in tool slot for water orb my carrying capacity dropped. Deciding to cast Strength of Atlas to counteract that I accidentally cast a ball spell (lightning ball IIRC), angering my pet who then proceeds to kick the crap outta my char... Krrrkk...

dieing of poisening in the water temple after killing all monsters getting the orb getting crowned by saccing the water snakes. I had loads of effiectient healing spells left and i had potions of cure poisends and 20 of the herb that cure poisen AND i had the ring of the high kings and various other artifacts :(

i had apearance 68 aswell (i have this thing about beauty)
I dont believe i let that orb gaurdian kill me!!!!!! i want my artifacts back!!!

My friend accidentally cast web on DL:49 and was stuck there for the rest of the game... couldn't move... couldn't cast spells.. couldn't do anything.

Stashing all my PoEH/PoUH before doing the blue dragon caves, then deciding on a whim to try to take on Sharad-Waador (in melee). 'It's ok, I can just pray for healing . . . oh . . . shit.'

My worst mistakes:

I entered to the pyramid with my lvl 15 dwarven weaponsmith. Didn't know back then that it had so many traps. I just ran over them, killed all mummies and jackal demons that tried to get in my way. I had about 3hp left when I saw the stairs. they were about 4 squares away from me.
First step. Nothing.
Second step. Nothing.
Third..." YOU ARE ENGULFED BY THE ROARING FLAMES!!!" NOOOO!!!

My worst blunder... hmmm...
maybe when I was playing a Trollish Beastfighter
about level 37. Hp over 500. I was in low Hp that time (Had just killed two emperor molochs) And i get a message; A stone block hits your head. My hp Dropped to 3. =D. Seconds after another Greater moloch appears (I had uden all my prayers and potions on the first two greater molochs) it hitted me (Only one way out from that room) and my hp dropped to -56. An hour after it i realized that i could have escaped it with my wand of Teleport.
Draclor Chain is my home, and I will protect it from chaotic plague bearers.

Ahh, i have one blunder. A YASD, if you want to use that term.

I had a lvl 23 human fighter and i had cheated(bad me) me a couple o' wishing wands with over 2 million charges. I had killed the Snake from beyond and the Karmic Wyrm @ ToEF. There were still tons of monsters in the top floor of the tower, so i couldn't get the orb! My solution? Plug my ears and summon a Banshee. It worked fine, until the Banshee died. But, being a resourceful little cheater i wished for a another one. Turned out, i had somehow managed to get the wax out of my ears.

"Hey wait a minute, i aren't i deaf anymore? HOLY F-CKING...!"

Inventory -> Tool -> Beeswax -> Exit -> Wail -> My heart got ripped to shreds. Yum.

Hehe - latest one:
I've stumbled to a casino shop and decided to identify all items (picking them up until overburdened, then using blessed scroll of identify, dropping unpaid - moving on)...
And was stunned when merchant said I owe merchant's guild say a... 183455 gold... (i have about 94 thouthand on me and no ability to teleport)
Well, shit happens, I guess. =(

something happened today that wasn't really a blunder on my part but was extremely annoying anyway:

I return to the bottom level of the puppy cave, long after having initially generated it sometime on day 3. I explore and find the puppy, then lead it into a small room which I carefully scan for traps before leaving the dog inside and locking the door. I then go off to clear out the orc vault next door.
a few steps later "You sense the death of an innocent being."
what the hell?
I go back into the room: "Something strikes at you from nowhere!"

Wand of ball lightning vs. Room full of gray oozes. I zap it because I don't want to corrode myself or any good weapons and gloves, yet I want them gone. The direction the wand chose was due north. I was in a tight, one-block space.

My blunders usually aren't deadly, and usually aren't too bad, but here's a couple recent examples:

My drakeling monk had 4 divine interventions instead of 2, because I accidentally read the weird tome and prayed to get rid of the confusion. That got me a bit frazzled... and I accidentally read the tome again.

Some Adom playing friends of mine have been bugging me to do an ultra ending, so I read up on them... and the biggest problem I'm having is all the alignment fiddling (and I'm not even anywhere yet). I went to NC from a Terinyo quest, but accidentally dropped things on a chaotic altar anyway... that was fine after I killed off the rats. So what I meant to do next was to sacrifice 1 coin to get back to C, but I somehow managed to drop things again. My blessed girdle of carrying was torched by the time the "do you want to continue dropping stuff?" message came.

Well, not fatal, but definitely a step towards becoming a paragon of stupidity.

1. Got poison hands corruption.
2. Rushed to my hidden stash with my only POCC.
3. Forgot I had thick gauntlets and likewise, did not equip them.
4. With a smile on my face, I quickly unlocked the door to my secret stash.
5. Eagerly picked up the POCC(thinking my problems would be solved)
6. Inadvertantly became the proud owner of a blessed potion of poison.

This, and playing with wands of ball lightning in confined spaces, will guarentee my ascention to a paragon of stupidity!!!!MWAHAHAHA!!

I was recharging some wands with my wannabe archmage (L50, had TotRR and other mana stuff, extremely pps) to drain more pps. Well, I was a bit bored with that and accidentally pressed just enter when I had to write amount of recharges. Suddenly I received a strange message and then: ' you die'. The game seemed to think that I was trying to charge my wand with 1 000 000 or so charges ;( , and the wand exploded fiercely bringing my hp from 700 to -300... Sigh...
The rust monster touches your blessed Trident of the Red Rooster. Nothing happens. You laugh at the monster's pitiful attempt.

I once dropped all stuff onto an altar with a low lvl wizard. When picking it back up I just held 'a' and watched the list go down.

Dropping everything after checking the contents of the HMV shop, a ratling duelist attacked me, casuing me to drop my adamantium scimitar... did you know being disarmed in a shop gives the shopkeeper the item for free!?!?!? i was so angry i attacked the ratling without watching my hp and he cut me up, and almost worse every weapon i equipped to fight him he disarmed me of and gave to the shopkeeper!!!! aaarrrgghhh!!!

accidentally zapped the blessed wand of fireballs in the dwarftown shop. i justr wanted to drop it to check how much is it worth:/
and my great trollish bard got slaughtered by the shopkeper and those muscular dwarfs

Was a playing a good game with a hurthling wizard. Had already got an AoLS through a RoDS and was set to go down from the Dwarven city to give it to Khelevaster - the first time I would ever have done this.

But first I had the final Thundrrar quest to do. I usually lure the demon over to the forge to kill Glod and then over to the temple to kiill the priest so that I have an anvil and can alter the altar without any negative effects.

Went to the temple first. The priest paniced and fled so the demon followed him but after a while it became clear the he wasn't going to finish him off. I decided to leave it at that and hope that I would be able to safely convert the alter.

Then went over to the forge. After ten minutes of teleporting around the forge and stat drains the demon still hadn't attacked the smith. So I started using bolt spells to take out the demon with the smith on the far side taking damage also. This worked surprisingly well. Until pressing the keys to quickly caused me top hit the smith without hitting the demon.

I hate Waldenbrook.

Hmm. I understand the forge thing but the altar? They are not that uncommon. And there is nothing worth saccing in dwarftown.

Blunder:

Well, once I wanted to sac a bit in dwarftown. Don't ask where I was looking at, but I was standing next to the Altar and the priest was on it.
He was not amused :)
